Laboratory Registration 實驗室註冊

***實驗室固定每年九月舉辦統一實驗室安全培訓並需通過筆試,合格者才可註冊成FULL USER。如申請者在年度其他時間註冊,只能註冊成TEMPORARY USER,直至通過安全培訓及考核者才可轉為FULL USER***

***This form is applicable only to users who have NOT taken ICMS annual laboratory safety training which usually takes place in September every year. Only those who completed the ICMS annual laboratory safety training and passes the examination, are competent to be registered as FULL USER. If applicants register at any other time over the year, he/she may ony be registered as TEMPORARY USER until ICMS annual laboratory safety training period.***

Instructions: 
This form is comprised of 6 sections: 

(1) Applicant detail; 

(2) Registration Lab: Please fill in either 6F or 7F. To register into both 6F and 7F, you will need to complete this form twice with one stating 6F and another stating 7F, and merge them into one pdf file while uploading.

(3) Experiment and support facilities; 

(4) Lab Training and skill: Please read through and be familier with this lab safety docmument  safety kit 2014.rar before selecting "Yes"

(5) For supervisor/responsible staff only: This section requires the names/signatures of one full user who have completed our safety trainng program and examination from your group, as well as your supervisor.

Please upload the completed form while creating your LMIS account.

***新註冊碩士生需另附以下文件以完成註冊流程

碩士生需完成240小時實習才可註冊成為實驗室臨時使用者,故必須先填妥以下三個文件:

1. Application letter for laboratory probationary training (Student's record)
   This form provides an agreement from your supervisor, which agrees you will be led by an assigned senior student from your group.

2. Application letter for laboratory probationary training  (Laboratory's record)
     A duplicate of the above form, but for laboratory's record only.

3. Registration form for laboratory probationary training
     A record showing you have reached our requirement for probationary training.

You can merge all completed forms into one pdf while uploading to the LMIS.


2. 正式使用者 FULL USER

當完成實驗室年度安全培訓並考核合格後,請下載及填妥下列表格,並登入本實驗室LMIS系統註冊新帳號及上傳表格:
After finished the annual laboratory safety training and passed the examination, please complete this form prior to using our laboratories and creating your LMIS account:

Instructions:

This form is comprised of 5 sections: 

(1) Applicant detail; 

(2) Registration Lab: Please fill in either 6F or 7F. To register into both 6F and 7F, you will need to complete this form twice with one stating 6F and another stating 7F, and merge them into one pdf file while uploading.

(3) Training and skill: Requirement #3 is only applicable to Master candidates, which requires a completeion of 240 hours lab practice (led by senior full users from your group). 

(4) Experiment and support facilities;

(5) For supervisor/responsible staff only: This section requires the names/signatures of one full user who have completed our safety trainng program and examination from your group, as well as your supervisor.

<<碩士生答辯及實驗室註銷流程>>

根據中華醫藥研究院教學委員會會議通過,本院碩士畢業論文答辯分別在每年1月及7月進行,流程如下(細節和日期請留意學院電郵公告通知):

1. 完成論文並經由論文指導老師向學院申請組成論文答辯委員會
2. 完成預答辯 

(中藥學專業同學必須在預答辯前完成有關實驗室的Check-out手續)

提交Laboratory check out form
-Check-out手續其中包括:

交還實驗室用具;書本、鎖匙、工具、色譜柱、其他實驗室資源等。如同學已分配使用空間,必須完全清理整潔方可。

-另外,亦需提交經導師確認的項目:包括實驗記錄本、藥材,提取物品和標準品的交接記錄。
-相關Check-out表格由實驗室職員以電郵方式通知答辯同學填寫。
-如同學在預答辯前沒有完成Check-out手續,辨公室將不為該生辨理答辯申請,煩請各老師及同學們注意。

3. 進行論文答辯

如需修改論文,請於答辯後一個月內提交論文最終定稿(必須得到指導老師確認為最終定稿)及相關資料。

4. 向學院辨公室職員(N22一樓1045室 Ada)提交以下資料(打印本):

A. 修改後的畢業論文定稿,建議釘裝6份,(其中3份送澳大圖書館、1份給本院、1份給指導老師、1份給作者本人);

B. 論文定稿的電子檔(pdf)刻錄在光盤內,並寫上姓名及學號;

C. 論文電子版提交表 (請於澳大圖書館網頁下載 http://library.umac.mo/html/forms/forms.html);

D.  自我聲明書 (GRS/Form/032,請於研究生院網頁下載 http://www.umac.mo/grs/en/download.php);

E. Confirmation of Master's Thesis Form(GRS/Form/033) (請於研究生院網頁下載 http://www.umac.mo/grs/en/download.php).

5. 向實驗室職員(N22一樓1041室Kio 電郵:SKKuong@umac.mo 或 N22一樓1045室Jeff 電郵:Jeffpong@umac.mo)提交電子版資料                                          

(請把所有電子版資料刻錄在光盤內,同時務必與指導老師確認論文、相關資料及數據均為最終版本,否則辨公室將不為有關同學提畢業申請):

A. 論文定稿的電子檔(pdf 及 word);

B. 答辯(ppt)及答辯時照的相片(jpg);

C. 學生的論文研究數據(rar)和發表文章(pdf);

D. 論文電子版提交表(pdf)(請於澳大圖書館網頁下載http://library.umac.mo/html/forms/forms.html);

特別提示:

各同學必須按中華醫藥研究院碩士論文格式編寫論文,有關資料可在中華醫藥研究院網頁下載.

另外,答辯後的同學必須向辦公室及實驗室職員提交以上資料,否則辨公室將不為有關同學提交畢業申請(辨公室不會另行再發任何提醒郵件).

希望各準畢業生注意提交資料的日期,以免影響畢業時間.

若未能於每年1月的答辯期間前完成以上程序的同學,其答辯申請將順延至7月進行。如有任何查詢,請主動致電中華醫藥研究院辦公室,電話 8822-4685。

<<博士生答辯及實驗室註銷流程>>

博士生答辯申請須按照學校既定的申請流程進行,詳情可詢問各自課題組老師。

在完成答辯前應完成實驗室註銷流程。

Before Oral Defense:
The following procedures should be submitted before 2 months of your oral defence date. (Deadline: 20th of every month)

1. The following forms should be submitted to the ICMS General Office when PhD students are ready for the oral defense:

a) Thesis Declaration Form (GRS/Form/032) - To be completed by the concerned student

b) Confirmation of PhD Thesis Form (GRS/Form/028)
- To be completed by the Supervisor and endorsed by the Chair of the Academic Unit's GSC/PC

c) Nomination of External Examiners for PhD Examination Committee (GRS/Form/31)
- to be completed by the Principal Supervisor and endorsed by the Chair of the Academic Unit's GSC/PC and submitted to the Graduate School when nominating the external examiners.

d) Form for Submission of PhD Thesis (GRS/FORM/034)
- To be completed by the concerned student

2. Each member of the Advisory Committee must provide brief recommendation for the student to do his/her oral defense

i) each recommendation should be around half page long with University letter hand and signature.
ii) the recommendation should include:
a) comments on the quality of the thesis
b) whether the thesis is ready / up to the standard for oral defence

3. The student has to submit a list of his/her publications and CV.

4. The student's thesis must follow the format governed by the Guidelines Governing the Format of PhD Theses. (Both the hard copy and the soft copy).

5. CV of all proposed external examination committee members.

6. For external examination committee members:
There should preferably be 2 external members in any single PhD oral defense examination committee. (1 external member is accepted) These external members should come from any 2 of none local University:

When all the above documents are collected, we can prepare the proposal for PC, GRS and Senate approval.

After Oral Defense:

The following items should be submitted to the ICMS General Office between one month after the PhD oral defence:

1.     Confirmation of PhD Thesis Revision (GRS/FORM/035) or (GRS/FORM/036)
       – to be follow the committee's decision.

2.     PhD Thesis Revision Form (GRS/FORM/037)

3.     Thesis Revision Summary Report
      – with signature both by supervisor and student.

4.     Revised Thesis
      – both 2 soft copies (1 copy thesis only; 1 copy including thesis/ppt/publication ect...), 4 hard copies with hard cover.

Graduation proposal will be prepare when student provided all the above documents after oral defense.

Reservation for High-End Mass Spectrometers

    If you would like to reserve the High-Resolution Mass Spectrometers in MS facility, please fill in the "MS Facility Request Form" and upload to intranet LMIS system for reservation.
